Job Description

QUALIFICATIONS STANDARDS

1. Education & Licensure:

• Bachelor’s degree of science in Nursing.

• Must be a PRC License.

2. Work Experience:

• Minimum of 6 months to 1 year of experience of probationary period.

• Must have at least 3 years with Hemodialysis training or certification

• Preferably with license as a Certified Nephrology Nurse (CNN) or accomplished Hemodialysis Training or certification.

3. Skills Set:

• Problem-solving skills – adept at identifying issues related to patient care, equipment malfunction, or staffing concerns and finding effective solutions.

• Decision-making skills – the ability to make sound and timely decisions is crucial.

• Critical thinking skills – must be able to evaluate patient responses, recognize complications early, and apply medical knowledge to adjust treatment plans accordingly.

• Computer Literate - the ability to navigate approved digital systems efficiently ensures accurate record-keeping and compliance with healthcare standards.

• Good communication skills (both oral and written) - able to convey instructions effectively, provide health education to patients, and document patient progress accurately in reports and records.

• Efficient in performing direct nursing care – able to monitor vital signs, initiating dialysis treatments, responding to emergencies, and ensuring that patients receive the highest standard of nursing care throughout their sessions on top of leading the team.

Specific Job Duties & Responsibilities:

• Synthesizes and endorses information from patient’s chart including medical histories, prescribed medications, vital signs readings, clinical observations, diet and other pertinent information to learn about the patient’s conditions, determine care priorities and prepare test/medicine requisitions according to physician’s specifications.

• Provides direct care to patients undergoing HD based on the rules and regulations established by the Department of Health (DOH) and approved company policies and procedures;

• Assesses patient using the nursing process before, during and after dialysis and documents the same accordingly. Takes and records vital signs accurately and regularly based on patient’s needs and indicate real time vital signs are taken. Information provided by vital signs graphs is utilized to assess patients’ conditions and determine the need for possible interventions;

• Prepares, administers and documents correct medications to assigned patients and observes patient reactions, signs and symptoms. Properly documents the same. Initiates remedial actions when appropriate and accurately records these in the chart.

• Participates in regular meetings as well as conferences, continuing education programs of the department and unit. Exchange information with colleagues and make suggestions to better improve treatment plans and implementing specific procedures.

• Uses professional judgment and critical thinking to prepare, prioritize and execute the patient's plan of care consistent with patient assessment data and physician's treatment plans and professional standards of evidence-based practice/care.
