Job Summary

The Patient Navigator serves as a key support resource for patients, helping them understand and access healthcare services, overcome barriers to care, and move smoothly through the healthcare system. This role focuses on care coordination, patient education, and advocacy to improve health outcomes and patient experience.

Key Responsibilities

• Guide patients through the healthcare system, including appointments, referrals, and follow-up care
• Educate patients and families on diagnoses, treatment options, care plans, and available resources
• Identify and address barriers to care (e.g., transportation, insurance, language, financial, social needs)
• Coordinate care among providers, specialists, social services, and community resources
• Assist with scheduling appointments, diagnostic tests, and procedures
• Support patients with insurance navigation, authorizations, and referrals
• Maintain accurate and timely documentation in electronic health records (EHRs)
• Advocate for patient needs and promote culturally competent, patient-centered care
• Track patient progress and ensure continuity of care
• Participate in care team meetings and quality improvement initiatives

Qualifications

Required:

• Bachelor's degree in Nursing with active PRC license
• Experience in healthcare, social services, case management, or patient support
• At least 1 year bedside experience, preferably with IVT experience and Medical Surgical experience
• Must be willing to work in the OPD
• Basic knowledge with Clinical Patient Navigation, Palliative Care, and Oncology
